I just fucked up big time.
I just pushed an IM dose of Ketamine via IV route. Pt is fine. Got tachycardia and HTN for like 45min and then came back down. Never lost airway. But I am so fucking screwed. I’ll be lucky if I have a job after this. I’m so fucking upset.
Edit: for some context the provider gave me a verbal for IV ketamine. Then the provider went into the order and changed it to IM, while I was on the phone with pharmacy trying to get the to process and send it up stat. The pt was in four points and thrashing and screaming and had already broke out of restraints twice and removed two IVs. And nothing was touching this pt. Phenobarb, Ativan, haldol, none of it even slowed this pt down for more than 10min. I was frantic to get the meds on board asap before the pt broke out again and removed my last IV. We also had some technical issues with our WOWs, so there were only two functioning that were divided among three nurses. So I wasn’t able to scan the meds before administering them, which may have just been one more opportunity to see that they were ordered for IM.
